11/5/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ashiyah. Upon arrival to the home, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. She stated to QMHP-C that she was having a good day and that she was tired because she went to bed late but she wanted to make sure that she had an early session because these are what keeps her from acting out of her character on the weekends. 2. Behavior: Ashiyah was cooperative throughout the session as she was being active and engaging fully in each part of the sessions. 1.Intervention: QMHP-C began the session by inquiring about Ashiyahâs current thoughts, feelings and emotions. QMHP-C discussed her mood and learning to cope with the different stressors that cause her to become unhappy. While processing with Ashiyah on those behaviors that cause Ashiyah to become unhappy, QMHP-C provided Ashiyah with positive self-talk. QMHP-C discussed with Ashiyah on reducing negative thoughts and positive decision making. 2. Intervention: QMHP-C discussed with Ashiyah on how her impulsive behaviors triggers negative consequences for her actions. QMHP-C engaged Ashiyah in coping skills to include thinking before acting, thought-stopping skills. QMHP-C taught Ashiyah that in any case thinking of acceptable and unacceptable behaviors before reacting supports dealing with impulsive behaviors. 1. Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided her to during the session. Ashiyah has difficulty with expressing her feelings and thoughts age-appropriately evident by her smacking her lips, rolling her eyes and stating to QMHP-C at times that she donât feel like talking about the same old thing over and over all the time because things are not going to change. As QMHP-C was discussing with Ashiyah about her behaviors, she constantly denies her negative interactions and behaviors. 2. Response: During the session, Ashiyah hunches her shoulders when she refuses to take responsibility for her actions. Ashiyah indicated that she do not think about the consequences of her actions because when she is mad she is mad and that is just that she is going to do what she wants to do. Ashiyah discussed with QMHP-C that id they say something that she donât like when she goes to court she is going to get mad and she is going to have an attitude and that was just that.
